Federally recognized Tribes operating an Indian health program operated pursuant to a contract, grant, cooperative agreement, or compact with the IHS pursuant to the Indian Self Determination and Education Assistance Act (ISDEAA), (Pub. L. 93 638). 3. Title V Urban Indian Health Program this includes programs currently under a grant or contract with the IHS under Title V of the Indian Health Care Improvement Act, (Pub. Law 93 437). 4. A Consortium of any of the above that can demonstrate a history of successful collaborative efforts in meeting goals and requirements of previous projects or grant programs. Smaller applicants are encouraged to apply as a consortium, especially if their diabetes registry is 250.
