Eligible Applicants Recipients agree that the benefits of the grant awards must be restricted to all or part of the of the Federal Emergency Management Agency (FEMA) declared major disaster states, which are Connecticut, Delaware, Maryland, Massachusetts, New Hampshire, New Jersey, New York , Ohio, Pennsylvania, Rhode Island, Virginia, West Virginia, and the District of Columbia. Eligible applicants and/or co applicants must be based or have significant operations in one of these states, including the District of Columbia, identified as a major disaster area. Non state applicants must partner with 1 or more State, Tribal, or Local public health agency in one of the states, including the District of Columbia, identified above as a FEMA declared major disaster area. Applicants will be required to attest (at the time of application) that funds requested will not be used for costs that are reimbursed by the Federal Emergency Management Agency, under a contract for insurance, or by self insurance2. 2. Self Insurance is a formal plan, pursuant to law or regulation, in which amounts are set aside in a fund to cover losses of specified types and amounts, typically by a commercial company. Appropriated funds are precluded from expenses that are or can be reimbursed by the formal self insurance plan.
