National organizations with health departments as their members are uniquely qualified to respond to the burden of chronic diseases in their communities through their vast experience of continuous implementation of evidence and practice based interventions, implementation of self management educational activities, and having the local, state, and territorial public health infrastructure in place to provide the proper foundation and support for programs that focus on reducing the chronic disease burden across the nation. These specific national organizations serve a cornerstone function for broad based public health activities in extending the reach of community based programs and accelerating progress towards the improvement of health outcomes related to chronic diseases such as obesity, diabetes, cardio vascular disease and stroke, cancer, arthritis, etc.
