A sponsoring facility includes any of the following 1.A hospital 2.A public health department 3.A community health center 4.A non profit health care agency 5.A school or school system 6.A program administered by the Indian Health Service or the Bureau of Indian Affairs or operated by an Indian tribe or a tribal organization.
