Tribal epidemiology centers Universities Urban Indian health organizations 2. Special Eligibility Requirements The applicant organization must include evidence of a scope of work and a mission to serve people with Attention Deficit /Hyperactivity Disorder or Tourette Syndrome. Documentation of 1) an organizational charter and mission to serve people with ADHD or TS, 2) a listing and CVs of medical or scientific advisory board that reflect expertise in ADHD or TS, and 3) a progress report demonstrating communication and outreach work with the target population, is requested, if not received the application will be considered non responsive and will not be entered into the review process. The applicant will be notified that the application did not meet the eligibility requirements . Documentation can be submitted by uploading in Grants.gov under Other Attachment Forms. Each document should be labeled (e.g., Proof of Organizational Charter, Proof of Public Health Mission , Proof of Medical and Scientific Advisory Board etc.).
