Services must be evidence based or evidence informed to the extent possible.Eligible applicants will be those entities that are well into the development of a dementia capable system and not those in the beginning of development of such a system. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement.
