Therefore, eligibility is limited to permanent supportive housing programs. The following permanent supportive housing programs are eligible to apply 1. HUD permanent supportive housing programs (Shelter Plus Care, SRO, SHP Permanent Housing, or SHP Safe Haven programs) or 2. Permanent supportive housing programs funded by non HUD sources. Applicants must verify their eligibility by providing the following documentation in Appendix 6 of the application. Grant awards will be made only to entities that provide appropriate eligibility verification. 1. HUD funded applicants must provide a copy of an award letter to verify a current, executed grant agreement from HUD. 2. Non HUD funded applicants must provide a letter from a comparable housing program funding source verifying a current, executed grant or contract agreement. The letter must include the following information 1. Brief summary describing funding source, including any funding requirements and/or restrictions 2. Amount of funding provided per year for applicant s permanent supportive housing program 3. Type of supportive housing (scattered site or facility based) and number of units provided 4. Amount program participants pay from their income toward housing and 5. Information about consumers choice in housing option in level and type of services received and tenancy rights (e.g. privacy in unit, leasing).
